FINANCIAL AND ESTATE INVENTORIES:
1. PERSONAL FINANCIAL AND ESTATE INVENTORY. This is a complete form used to assemble all of your financial and estate information. This form has lines which can be filled in by hand after the form is printed out.
2. ESTATE PLANNING INVENTORY. This is a form that you can use to assemble all the information your attorney will need to prepare your Will and other estate documents.
DOCUMENT PORTFOLIO:
1. DOCUMENT PORTFOLIO CHECKLIST. A Document Portfolio can be a standard notebook that you put together to keep a copy of all of your important papers in one place. In the Document Portfolio you can not only include your estate documents but also the following forms included on this resource disk;
2. PERSONAL PROPERTY MEMORANDUM. This document accompanies your will and allows you to designate items of personal property to beneficiaries.
3. PROFESSIONAL ADVISORS AND LOCATION OF DOCUMENTS. This form can be used to designate you professional advisors and the location of important documents.
4. INSTRUCTIONS FOR SUCCESSOR TRUSTEES. These instructions outline the responsibilities for someone who will become the trustee of one of your trusts at either death or disability.
5. PROCEDURES WHEN SOMEONE DIES. These procedures tell a personal representative what to do when someone dies.
6. A RECORD OF LIFE INSURANCE. Keeping track of your life insurance is very important. If your beneficiaries do not make a claim the company does not have to pay.
7. EXECUTION INSTRUCTIONS FOR ESTATE DOCUMENTS. Some forms require a notary others require witnesses and the will has a specific statutory procedure that must be followed.

1. WILL CODICIL FOR MAN. A Codicil is an amendment form for a Will.
2. WILL CODICIL FOR WOMAN. Since the terms for a man and woman executing a Codicil are different separate forms have been included.
3. REVOCABLE LIVING TRUST AMENDMENT FORM. You can make changes to your revocable trust using this form.
4. MEDICAL POWER OF ATTORNEY. This form allows your chosen representative to make medical decisions for you if you can’t make them for yourself.
